Our office hours are Monday - Friday 8:30am - 5:00pm.

Appointments

To make an appointment, please call us during regular office hours at
(540) 382-6613

Please bring the following items with you to your appointment:

• Completed patient registration forms
• Medication bottles for all current prescriptions (each visit)
• A list of over-the-counter medications and vitamins.
• Your medical insurance card(s)
• Previous tests such as X-Ray films (no CDs), MRIs, CT Scans, Mylograms, etc.

All new patients must have X-Ray's that have been taken within 1 month. If you do not have these, you may contact our office and x-rays can be ordered for you prior to your appointment. Each patient will be responsible for bringing their own films with them. Your appointment may be rescheduled if we do not have access to the actual films.

If your insurance requires a referral, we must receive this prior to your visit with us. It is also helpful if you bring a copy of the referral from your insurance company.

Cancellations

If you cannot make it to your scheduled appointment, we ask for at least 24 - 48 hours notice.

Prescriptions

Prescription refills should be handled at the time of the patient's regular scheduled appointment. However, should the need arise for a prescription refill between appointments, we will require a 24 hour notice.
